norepinephrine vs epinephrine

 

 

 

 

 

 

아나필락시스( anaphylaxis)

-> epinephrine 0.3@ IM + NS 1L full drop 하면서 30분마다 페니라민 1@, 덱사메타손1@ IVS로 계속.

 

심정지 (cardiac arrest)

-> 30:2로 CPR 치면서 4분마다 1@ IVS로 계속.

 

 

 

cardiogenic shock / septic shock으로 인한 저혈압과 서맥 증상

-> atropine 2@ ivs로 시도해보고 안 되면

-> 5% DW 500ml + norepinephrine (큐프린 주 8mg/4ml) 32mg=4@ 하여 3-30cc/hr 속도 IV로 주입.

 

 

 

 

 

 

단!! pulmonary embolism일 경우, IV fluid만 달고 대학병원으로 전원.

 

PE이면서 혈압떨어지면 NE 시도해 보기도 하나 혈관수축시켜서 PE 막힌거 악화시키고,

tPA도 뇌출혈 리스크 있기 때문.
